For the first 30 days, our caring team coordinates your care, connects you with resources, and ensures everything runs smoothly. This includes check-in via phone, telehealth, or in-home visits, plus education for you and your family on managing medications, daily activities, and follow-up needs. It's all about making sure you feel confident and supported while aging well at home.
You're eligible if you've been discharged from a hospital, skilled nursing facility, or similar inpatient setting and are heading back to your community-like your own home, assisted living, or with family. Our services are ideal for those with moderate to high medical needs, such as managing chronic conditions, new diagnoses, or changes in treatment.
